Printable Survey Page
We’ve got you covered if you prefer to distribute the departmental inclusiveness survey in paper form! Follow these steps to print out a paper copy of the survey:
- Download the DIS Paper Version [PDF] from our website.
- Print out the survey and distribute pages 1 and 2 to your department. Page 3 is for your use only to score the responses.
- Encourage everyone to complete the survey and return it to you.
- When collecting the surveys, we recommend allowing people to turn them in on their own time in a way that maintains anonymity.
- Once you’ve collected all the surveys, input the data into a spreadsheet or other program to analyze the responses.
