Google Forms Survey Page
We’re thrilled that you’re interested in learning more about using our survey to promote inclusivity in your department. Unfortunately, Google Forms doesn’t allow us to post a Google form as a shareable file. But if you’d like to use Google Forms to distribute the survey, here’s how:
- Contact Al Bardi at cabardi@sewanee.edu to request the survey to be shared with you as a Google form.
- You must have a Google account and either use your Gmail address to make the request or provide your Gmail address for sharing.
- You will receive your copy of the survey in Google Forms, and you will also receive a Google sheet that will score the Google Form responses for you.
- Additional instructions will be provided in the email you receive notifying you of the form and sheet being shared.
After collecting data, you can discuss with your department about the results and steps to improve departmental inclusivity in a way that is specific to your department. You can find more information on how to do the assessment process at Improving Departmental Inclusiveness.
